FPRP Cognitive Assessment Template

FPRP’s Care of the Older Adult Frailty Continuing Professional Development Program has been ongoing since 2019. Program content is designed and delivered by an expert team of Care of the Elderly trained Family Physicians and Geriatricians from across the province. To build on this great work the Frailty team has begun collaborating with eDOCSNL on the development of EMR tools to support frailty work within Primary Care. The first outcome of this collaboration is the Cognitive Assessment template.
